Terms of Service
The basic rules for using FormPilot.
Effective date: June 2, 2026
These Terms of Service govern access to and use of FormPilot. By creating an account, joining a workspace, publishing a form, or using FormPilot, you agree to these terms.
The Service
FormPilot provides tools for home-service businesses to create quote request forms, collect submissions, review responses, generate OpenAI-powered summaries, send notifications, use integrations, and manage related workspace data.
Accounts And Workspaces
You are responsible for keeping your account secure and for managing who has access to your workspace. Workspace owners and admins are responsible for the forms they publish, the data they request, the team members they invite, and the third-party integrations they connect.
Customer And Homeowner Data
You are responsible for collecting information lawfully and using it appropriately. Do not use FormPilot to request highly sensitive information that is not needed for a quote request. If a homeowner submits information through your form, you are responsible for your follow-up, use, export, and deletion practices.
Acceptable Use
Do not use FormPilot to send spam, collect information deceptively, upload illegal content, violate privacy rights, interfere with the service, attempt to access another workspace, reverse engineer private systems, or bypass plan limits, rate limits, authentication, or security controls.
AI Summaries
AI summaries are provided to help users understand and prioritize submissions. They are not guaranteed to be complete, accurate, or suitable for legal, financial, insurance, safety, or emergency decisions. Review the original submission before relying on a summary.
Integrations
FormPilot supports workspace custom domains, webhooks, Slack incoming webhook notifications, Google Sheets sync, Zapier-compatible webhooks, and read-only API keys. If you connect a domain or integration, you are responsible for the DNS records, destination, permissions, automations, and third-party account settings.
Billing And Plans
Paid plans, trials, subscriptions, invoices, payment methods, and customer portal flows are handled through Stripe. Plan limits may affect responses, storage, AI summaries, integrations, analytics history, watermarking, team access, and advanced features. If payment fails or a plan is canceled, access to paid features may be limited or disabled.
Uploaded Media
FormPilot may allow workspaces to collect job photos or short videos through public forms. Workspace members are responsible for using uploaded media appropriately and only for legitimate quote, service, support, or business purposes.
Changes To The Service
FormPilot is improving quickly. We may add, change, limit, or remove features to improve reliability, security, usability, or business fit. We will try to avoid disruptive changes, but we cannot guarantee every feature will remain unchanged.
Suspension Or Termination
We may suspend or terminate access if a user or workspace violates these terms, creates security risk, abuses the service, fails to pay required fees, or uses FormPilot in a way that could harm customers, homeowners, FormPilot, or third parties.
Disclaimers
FormPilot is provided as a business workflow tool. The service is provided without warranties of uninterrupted availability, error-free operation, or specific business results.
Limitation Of Liability
To the maximum extent allowed by law, FormPilot is not responsible for indirect, incidental, special, consequential, or punitive damages, or for lost profits, lost revenue, lost data, or lost business opportunities.
